Please call us to get your passcode if you are a previous patient. You can even fax us your forms or drop them off prior to your appointment to speed up your first visit.
Payment & Insurance
We’ll verify benefits before your visit and explain any out-of-pocket costs up front. Apple Eye Care accepts many plans, including: Tricare, Blue Shield, New Mexico Medicaid, VSP, Superior Vision, and OptiCare. If you don’t see your plan listed, please call—we may accept it.

Can I fill out forms on my phone?
Yes—our online forms are mobile-friendly. If you prefer paper, arrive 10–15 minutes early so we can help.
Do you take my insurance?
We accept many plans (including Tricare, Blue Shield, New Mexico Medicaid, VSP, Superior Vision, OptiCare). If your plan isn’t listed, call us—we may accept it.
Is there a fee for contact lens fittings?
Often yes. Contact lens exams include extra measurements, trial lenses, and training. We’ll review any fitting fees before we begin.
How do I send previous records?
You can bring printed records to your visit, or ask your previous provider to fax them to us. Call for details.
